What to Expect During Your First Visit

Here is what you can expect before your first visit with us:

• Please complete the Colorectal & Pelvic Reconstruction intake form. Once the form and other requested medical records have been received we will review your child’s information. Then you will be contacted to schedule your child's appointment.
• After you have set up your child's appointment, you will receive an itinerary with information specific to your child including:
  
  • Date, time and location of appointment
  • Any diagnostic testing that will be done
  • Potential surgery dates with various specialists

Children must be accompanied by a parent or legal guardian when coming for their evaluation at the Division of Colorectal & Pelvic Reconstruction.

• Please bring your child's insurance card and information. Co-pays will be collected at this time if required.
• Please bring a list of current medications and any diagnostic tests or medical records.

• Plan to arrive 15 minutes prior to each of your child's scheduled appointments to allow for parking and registration. Check your itinerary to see your child's schedule of appointments.
• Your Colorectal & Pelvic Reconstruction appointment may include seeing multiple medical providers. This often results in a longer clinic visit than other medical appointments. For this reason we recommend bringing a favorite toy, snack, or activity to help your child cope with this extended visit.
• Appointments may occur over the course of one to five days. Be prepared with any necessities your child may need such as snacks, diapers, formula, etc.
