- Define health care transition and related terminology, concepts, and scope of the problem for youth with complex chronic health conditions.
- Identify potential quality and safety related issues around caring for transition aged youth in pediatric settings.
- Describe and utilize data supporting models of care for this population and how other health systems are responding to this issue.
- Identify opportunities in our community/health system neighborhood to improve health outcomes and position as a leader in implementing processes and policies for the transition aged youth we care for.
