As the nurse navigator, Harold coordinates cardiology referrals from physicians and health care facilities, provides comprehensive patient education to families and ensures continuity of care with service excellence as a priority.
- A single point of contact for referring providers. As the primary point of contact for referring providers, Harold can obtain and send documentation and images, as well as assist with clinical questions.
- Care coordination. To help patients navigate the system, Harold will manage appointments for families to ensure that they are scheduled with their desired specialist and seen in the appropriate timeframe, while coordinating other specialty appointments.
- Patient education and follow-up. Harold will provide support for patients and their families after discharge from cardiology services. This includes ensuring follow-up appointments are made and reviewing medications.
The nurse navigator’s primary focus is urgent or complex patients. Routine scheduling should still be handled through the call center (1-888-884-BEAR). If any concerns or difficulties arise with making an appointment, please contact Harold.
