New Cardiology Nurse Navigator to Facilitate Care for Your Patients
Charles Fleming, B.S.N., R.N., C.P.N., steps into the role of nurse navigator at the Children's National Heart Institute, facilitating care for complex and urgent cardiology patients.

As nurse navigator, Charles will coordinate cardiology referrals from physicians and provide comprehensive patient education to families while ensuring continuity of care. The nurse navigator's primary focus is urgent or complex patients.
- A single point of contact for referring providers. As the primary point of contact for referring providers, Charles can facilitate with documentation and images, and assist with clinical questions.
- Care coordination. Charles will manage appointments for families to ensure that they are scheduled with their desired specialist and seen in the appropriate timeframe, while coordinating other specialty appointments and helping families navigate the system.
- Patient education and follow-up. Charles will provide support for patients and their families after discharge from cardiology services. This includes scheduling follow-up appointments and reviewing medications. Routine scheduling should still be handled through the call center (1-888-884-BEAR).
