Applications for the fellowship should be submitted through the SF Match. Active military personnel may apply outside the usual SF Match cycle. In addition to the application, we also need the following:

Three letters of recommendation from three physicians (one of whom must be the chair or program director of the department where you trained)
Curriculum vitae
Personal statement
ITE scores
USMLE scores (1-3)
ECFMG certificate (if applies)
