Research Interests
Nina Deutsch, MD, focuses her research on the physiology and pharmacology of dexmedetomidine, a new and highly specific alpha-2 agonist. Dexmedetomidine has the potential to attenuate opioid tolerance, provide analgesia, and inhibit the sympathetic stress response more profoundly than is possible with opioids.  Currently there is very minimal published data concerning pediatric pharmacokinetics, pharmacodynamics, and pharmacogenomics of this drug. Its full characterization may allow for its widespread application in infants and children, obviating the need for modulation of opioid induced morbidity. In the pediatric cardiac population, greater understanding of the drug's hemodynamic and respiratory effects as well as its attenuation of the stress response will dramatically change future management of these patients. Dr. Deutsch’s study investigates the pharmacokinetics and pharmacodynamics of dexmedetomidine in three patient populations: patients with a transplanted heart; patients who have undergone bidirectional cavopulmonary anastomosis; and patients with normal physiology. Additionally, Dr. Deutsch will attempt to define the dose-ranging, MAC-sparing effect of dexmedetomidine in these populations. The study is being performed in patients undergoing cardiac catheterization, allowing for comparison of various hemodynamic parameters using dexmedetomidine versus general anesthesia and controlled positive pressure ventilation, the current standard practice.
