Preparing for Your Outpatient Appointment

There are a few things you can do to prepare for your appointment at one of Children's National Medical Center's regional outpatient centers:
  • Get directions to the location designated for the child's appointment.
  • Arrive 15 minutes ahead of your scheduled appointment time for registration and check-in.
  • Bring the following items to your visit:
    • Photo Identification - for the adult accompanying the child. Please Note: The parent or legal guardian must provide written permission in order for anyone else to accompany the child to the appointment.
    • Insurance Card - Learn more about insurance and billing.
    • Physician Referral - If required by your insurance.
    • Financial Assistance Paperwork - where applicable. Learn more about financial assistance.
    • Completed Forms - If your child has an appointment in one of the following specialties, please print and complete the forms listed under the specialty below, prior to your visit.
