ECFMG certificate (if applicable)

Signed Confidentiality & Orientation Agreement Sheet
Signed ID, Building & Parking Form
Completion of TRACKS (Cerner) Training and Assessment

If you have any questions regarding the credentialing process, contact the Medical Education Office (202-476- 3950). If you have questions concerning the logistics of your rotation, contact the Children’s National department coordinator.
Multiple Rotations – Trainees who return for more than one rotation in a given academic year only need to complete one application per academic year.
