Plan your child’s Physical

For children over the age of 2, we recommend a full physical exam annually.  (Infants are seen more frequently, based on the recommended schedule below).

What is included in a Physical Exam:

A physical exam is a comprehensive health prevention and health maintenance evaluation.  Age-appropriate vital signs, growth assessments, developmental screenings, nutritional assessments, interval family and social histories, preventative safety screenings, and other age-appropriate health screenings are performed.  As well, a full physical examination is performed.  Immunizations are updated when appropriate, laboratory evaluations are ordered when indicated, and anticipatory guidance is offered.

Please bring any forms for school or child care settings with you to your appointment.  For public school children in Montgomery county, we have links to downloadable health inventory and medications forms in the Forms section of our website.

For those families who only become aware of a form requirement after a physical has been completed, please drop off the form at our office to complete.  Please allow 1 week notice for form completion.

For children under the age of 2:
Infants are seen for preventative “well baby” examinations on the following schedule:
  • Initial newborn exam (typically within a few days of discharge from the hospital or birthing center)
  • 2 weeks old
  • 1 month
  • 2 months
  • 4 months
  • 6 months
  • 9 months
  • 12 months
  • 15 months
  • 18 months
  • 2 years.

Well baby examinations are inclusive of appropriate assessments of nutrition, growth, development, health and safety assessments, infant care guidelines, and immunizations.
